Yoga Class Participation Waiver & Release of Liability
By RSVPing or signing in, I acknowledge that I have read this waiver in full, understand its contents, and agree to its terms.
I acknowledge that yoga is a physical practice that involves movement, stretching, balance, and physical exertion, and that participation in yoga classes involves inherent risks, including but not limited to muscle strains, sprains, aggravation of pre-existing conditions, falls, or other injuries.
I understand that I am voluntarily participating in this yoga class and agree to take full responsibility for my own well-being during and after class. I understand that I may stop or rest at any time and that it is my responsibility to listen to my body and only participate to the extent that feels safe and appropriate for me.
I acknowledge that yoga is not a substitute for medical care, examination, diagnosis, or treatment. I affirm that I am physically fit to participate and have consulted with a physician if I have any medical conditions, injuries, or concerns that could affect my participation. I agree to inform the instructor of any serious illness, injury, pregnancy, or physical limitation prior to each class.
In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless the yoga instructor, Philadelphia Church, Bella Terra, Inc., and any affiliated owners, employees, volunteers, or representatives from any and all claims, demands, causes of action, or liability for injury, loss, or damage to person or property arising out of or related to my participation in this yoga class, including those caused by negligence, to the fullest extent permitted by law.
I understand and agree that this release applies to all present and future claims arising from participation in yoga classes offered at this location or by this instructor.
I agree to indemnify and hold harmless the released parties from any claims brought by or on behalf of myself or any third party arising from my participation.
For participants under the age of 18, a parent or legal guardian must sign, acknowledging and agreeing to the terms of this waiver on the minor’s behalf.

Photo & Communication Consent (Optional)
I grant permission for photographs or video taken during class to be used for Yogibelle promotional purposes, including the website or social media platforms. I understand that photos are used minimally and typically do not include identifiable faces.
I understand that participation in photos is voluntary, and if I do not consent, I may indicate this on the sign-in sheet. I also understand that consent may be withdrawn at any time by notifying the instructor.
I consent to being added to an email list for weekly updates and yoga schedule information. I understand that I may unsubscribe at any time.
